STEP 2:
Fill out forms

You should complete the DS-156 EVAF form electronically and then print the form on one page.  With the new process, the system has barcodes which coincide with each applicant.

Under no circumstance will the Consular Section accept DS-156 requests made by hand or without the corresponding bar codes.  The bar codes are extremely important; nothing can be processed unless they can be read correctly.

After the forms are completed with the correct bar codes, the applicant should complete the DS-157 form by hand.  If you are requesting a student/exchange or investment Visa you should also complete:

• Additional Form DS-158
  (This form is only for people who would like to obtain an F, M or J Visa)

This form should be printed and completed by hand.  The only form that should be completed electronically is the DS-156 EVAF.
