STEP 3:
Organize Documents

Business and Tourist Visas (B1/B2), are the most requested Visas, therefore there are specific items that you must present.  Should you want another Visa you should consult the table for more information and the additional requirements.

  1. Valid passport (Effective on April 2, 2009, all visa holders traveling to the United States must have passports with a validity of no less than six months after the date of exit from the United States);
  2. All previous passports;
  3. One (1) recent 2"x2" photograph in white background taken within the last six months.  We do not accept photos that are retouched or altered;
  4. Payment's receipt of US$131.00 of Processing Fee from the Banco de Guayaquil;
  5. Completed form DS-156 EVAF with the corresponding bar code page;
  6. Completed form DS-157 (print and fill out by hand);
  7. For applicants under age 18: Both parents, even if they are separated or divorced, or who has the legal custody of the child, must appear at the interview with the child. This must be verified by the presentation of the documents such as power of attorney. The parents or the person having legal custody of the child  must present two ID's with pictures issued by the government of Ecuador in case of Ecuadorian citizens or issued by the country of citizenship, (ie: national ID, passport, driver’s license). Also, you must present a certified copy of the "registro de nacimiento" from the actual registry book and the authorization to leave the country issued by the “Tribunal de Menores” or by a notary. Please bring original and copy of both documents.

Supporting documents that can demonstrate employment or student status:

  • Employment certification, showing salary and time of service
  • Recent pay invoices and salary receipts
  • A copy of your university degree/status

Supporting documents that can demonstrate economic stability:

  • Original bank and credit card statements for the past year.  We do not accept bank "certificates."  Please bring 6 to 12 months of statements showing accounts movements
  • If you own property: Statements defining what you own
  • Proof of rent payments for the past 3 years
  • Tax payer Id

Supporting documents that can demonstrate familial bonds:

  • Certificate of identity

  • Certified photocopy of birth certificate

  • Marriage Certificate

  • Divorce Certificate

  • Death Certificate
