VISA CLASSIFICATIONS
Crewmembers (C1/D)

The C1/D Visa permits members of airline or shipping crews to go to the United States for short stays while on business.

The members of shipping crews must present a letter from their company, which must specify their position, the duration of their contract of employment, the name of the boat, the date they boarded and a signed legal authorization.

Necessary Requirements for this type of Visa:

  1. Valid passport (which will not expire in 6 months);
  2. All previous passports;
  3. Birth certificate if the applicant is younger than 18 years of age;
  4. One 2"x2" recent passport photograph on white background;
  5. A receipt of payment for 131.00USD from the Banco de Guayaquil;
  6. Completed form DS-156 EVAF with the appropriate bar codes (electronically);
  7. Completed form DS-157 (printed and fill out by hand);
  8. Employment Contract (for members of shipping companies only);
  9. A Labor Certification (for airline crews only).

Applicants for this type of visa must come to the Embassy at the specified date and time given by the call center.
