Depending on the type of travel you plan to perform, you would need to meet all the requirements depending on the type of the visa you apply.

Basic Requirements:

1. Valid passport (Effective on April 2, 2009, all visa holders traveling to the United States must have passports with a validity of no less than six months after the date of exit from the United States);
   
2. All previous passports;
   
3. One (1) recent 2"x2" photograph in white background taken within the last six months. It must be free of shadows or glossy finish, we do not accept photos that are retouched or altered and it MUST comply with all the photo requirements;
   
4. A receipt of payment of the Processing Fee from Banco de Guayaquil for $ 131.00;
   
5. Completed form DS-156 EVAF with the corresponding bar code page (the form MUST be laser printed and of optimal quality);
   
6. Completed Supplemental Nonimmigrant Visa Application form DS-157 (print and fill out by hand);
   
7. Birth certificate if the applicant is less than 18 years old.
