In Person:  Bring your passport to the ACS Unit any time during our opening hours for passports processing. Fill out form DS-4085; this form should only be used to add visa pages to a previously issued, currently valid U.S. passport.  No appointment is necessary. There is no fee. Processing will take approximately one hour.

Third Party: If you wish to have a third party bring in the passport and the completed and signed form DS-4085 on your behalf, you should also give him/her a letter of authorization in which he/she is named and he/she should carry his/her own passport or Ecuadorian cédula for identification.
