U.S. passports

Amendments  

Passport Services will no longer amend valid passports. Instead, customers requesting name changes, extension of validity for limited passports, or correction of a printing error will apply for replacement passports.

If the change is within one year after issuance, you will need to bring:

  • Two recent 2"x2" color photos with white background. Make sure that the pictures comply with the requirements.
  • U.S. Passport Re-Application Form DS-5504.
  • Passport that needs to be changed.
  • Documentation supporting the amendment.
  • Cash or Credit Card to pay the corresponding fees.

 

If the change is more than one year after issuance, you will need to bring:

  • Two recent 2"x2" color photos with white background. Make sure that the pictures comply with the requirements.
  • Completed passport application forms
    DS-82 - For adults.
    DS-11 - For minors (16 years old or younger) or if is your FIRST ADULT PASSPORT.
  • Passport that needs to be changed.
  • Documentation supporting the amendment.

 

Fees may be paid by cash, credit card (Visa, MasterCard, American Express, Discover and Diners Club) or certified checks from an American bank.
NO PERSONAL CHECKS ARE ACCEPTED.
